Therapeutic Phlebotomy Policy
Pricing:
- Established Patients: Each standard therapeutic session is $80.00.
- Non-Patients: Each standard therapeutic session is $95.00.
Additional Costs:
- The standard therapeutic phlebotomy session includes up to two collection bags. In cases where more are required due to clinical necessity, “thick blood” or other issues, each additional bag will incur a $25 fee. This is to cover the extra materials and time needed for your procedure.
Patient Requirements:
- Established:
- Your Aura Vida Health and Wellness provider will provide your therapeutic order.
2. Non-patients Requiring Therapeutic Phlebotomy:
- You must provide:
- Therapeutic Phlebotomy Policy
- A copy of your most recent lab results, dated within the last 90 days, indicating your hemoglobin and hematocrit levels.
- Documentation from your provider recommending a therapeutic phlebotomy or blood donation status.
- You are required to:
- Sign the Aura Vida Health and Wellness Medical and HIPAA consent forms.
- Create a patient portal account to upload your documents before your appointment.
Payment Policy:
- Payment will be collected for all patients at the end of your appointment.
Important Note: Failure to complete all required steps prior to your appointment may result in rescheduling or cancellation of your session.
